How to Register
If you wish to register with this practice, please visit the surgery and request a registration pack. You will be asked to complete a new patient questionnaire and show one form of photo ID.
- Registration is with the practice rather than an individual doctor
- You are, however, free to see the doctor of your choice but may have to wait longer for an appointment with your preferred GP
Acceptance on to the list is as at the discretion of the Doctor and we may ask you for proof of identity.
Practice Area
The practice ‘area’ includes St Albans City, London Colney, Colney Heath, Sandridge and Chiswell Green. Anyone living within this area is welcome to apply to register as a patient.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
